About the Role
Key Responsibilities
As a Client Liaison at Ozcare, you will support the delivery of high-quality care services in alignment with aged care standards and internal policies. Your responsibilities will include:
- Providing information and guidance to new and existing clients
- Conducting client intake assessments and planning personalized care
- Adjusting care plans based on evolving client goals
- Completing required intake documentation using approved tools
- Delivering advice on care planning to support positive client outcomes
Required Skills and Experience
To succeed in this position, you should demonstrate:
- The ability to assess, plan, and evaluate care with clients and families
- Familiarity with community-based services and referral pathways
- Good computer skills including email, Excel, and data entry
- Strong time management and organizational abilities
